Hospital & Hospice Bedside Notary in Birmingham

When someone you love is in the hospital or on hospice, important paperwork can’t wait for them to make it to a notary’s office. We come to the bedside and handle the notarization calmly, quickly and correctly, so your family can focus on what matters.

Where we come to you

  • UAB Hospital and the Birmingham medical district
  • Children’s of Alabama
  • UAB St. Vincent’s Birmingham
  • Grandview Medical Center
  • Baptist Health Brookwood
  • Hospice houses and inpatient hospice units
  • Rehab centers, nursing homes and assisted living
  • Private homes, including patients receiving hospice care at home

Service area: Birmingham and other parts of Jefferson and north Shelby Counties. We’re an independent notary and am not affiliated with any of these facilities.

Documents we commonly notarize at the bedside

  • Durable (financial) powers of attorney
  • Advance directives, living wills and health care proxy forms (when a notary is wanted — see witness rules)
  • Self-proving affidavits for wills
  • Affidavits and sworn statements
  • HIPAA and medical-records authorizations
  • Real estate and vehicle documents that need an acknowledgment
  • Parental consent forms

How it works

  1. Reach out. Call 334-645-4134 or use the contact form. Tell us the facility and room, what kind of document it is, and how soon you need it.
  2. We get ready. We’ll confirm your fee estimate, check that the signer has an acceptable ID, and ask whether witnesses are needed. It helps to ask the nurse when the patient is usually most alert.
  3. At the bedside. We verify identity, make sure the signer understands the document and is signing willingly, watch them sign, then complete the notarial certificate and a journal entry.

What to have ready

  • The complete document, unsigned. Please don’t sign ahead of time, and fill in all blanks before we arrive.
  • The signer’s current, government-issued photo ID — driver’s license, state ID, passport or military ID. A hospital wristband is not ID. If the ID is at home, ask a family member to bring it.
  • Witnesses, if your document requires them. we can help arrange independent witnesses.
The signer needs to be alert and able to communicate. Before notarizing, we have to be satisfied that the signer knows what they are signing and is doing it of their own free will. If they are heavily sedated, confused or unable to communicate their wishes at the visit, we can’t go ahead. This protects them and makes sure the document holds up. If that happens, we’ll tell you right away and we can try again at a better time.
